Checklist before your appointment

There are some critical things you need to do before coming to your appointment.  It will save you time and speed up the assistance we can provide to you.

Checklist for organising a Student Access Plan

  1. Collect or download a “Verification and Impact Statement” (VIS) document
  2. Make an appointment to see your Medical Practitioner
  3. Ask the Medical practitioner to complete the VIS document
  4. Make an appointment to see the Disability Advisor (DA) to discuss the contents of the completed VIS and to discuss your situation and possible accommodations to your study
  5. Bring the VIS and related documents to your appointment
  6. Bring along your SATAC/ Year 12 accommodations if they are relevant
  7. Bring your timetable
