Elite Athlete Registration Form

To be eligible for the services outlined on this website, you must register with EASIS each year.

Complete all the fields in the following online form before selecting the 'Submit' button or you'll have to re-enter all your details again. It takes about 10-15 minutes to fill out the form properly. If you encounter problems with the form, we suggest you copy and paste the fields into a Microsoft Word document, save it and attach it to email addressed to eliteathlete@adelaide.edu.au.
