Q: How do I get a replacement certificate?Answer:If your original degree parchment is lost, destroyed or damaged you can request a replacement parchment. You will need to fill out a form to request the replacement certificate. You can download the form from the Graduations web site. The replacement will cost $100 and you will need to submit a Statutory Declaration signed by a Justice of the Peace stating the circumstances in which the loss, destruction or damage to your original occurred. The damaged original parchment will need to be surrendered to the Graduations Office. The replacement parchment will not necessarily look like the original as it will be issued in today's format for parchments, and it will say that it is a copy. Your name on the parchment will be printed the same as it was on the original even if you have since changed your name. If you have changed your name, you can request your parchment to be re-issued in your new name. You can download the request form from the Graduations web site.
