Instructions for Completion of Overtime/Additional Hours Claim Forms
- This form should be used in respect of claims for payment for:
- 1.1 Overtime, i.e. time worked in excess of eight hours in any one day or in
excess of the specified weekly hours of duty for a full-time employee engaged
in the relevant classification, in accordance with the Enterprise Certified Agreement.
- 1.2 Additional hours worked by part-time staff, i.e. additional hours which do
not exceed:
- eight in any one day, or
- the specified weekly hours of duty for a full-time employee engaged in the
relevant classification. Such work is remunerated at the appropriate ordinary
rate.
(Note: No part-time employee shall be employed for less than three hours in any
one day.)
This form must not be used for payment of Casual employees.
- In order to meet a statutory obligation and to facilitate calculation of
penalty rates the “Record of Working Hours” section must be completed.
- One copy to be sent to Payroll Services. One copy to be retained by Department.
