Macintosh Personal Computer Network Access Request Form

This service is provided via the ITS Helpdesk. This service is provided based on a fixed hourly rate of $130. More complex requests may need to involve an additional fee if the total time spent exceeds 1 hour.

Personally owned computers will be granted access to Web Based Email, Internet and Printing only. There is no access to University File Servers

Once you have submitted this form the following process will occur:

  1. The form will be forwarded to the ITS Helpdesk for action.
  2. On the day ITS helpdesk receives the approved request they will contact Next Byte
  3. Next Byte will contact the user within 48 hours to arrange a time.

Please read the following information:

I agree that ITS can provide additional support for my computer through Next Byte at the rate of $130 per hour.

I agree that I will follow all instructions provided to me by Next Byte and the ITS Helpdesk on how to keep my computer updated against Viruses. I also understand that if my computer becomes compromised (infected by a virus or is exposed by a vulnerability) ITS may need to remove it from the network to protect the University network.

I also agree that repairing my compromised computer will be at my cost.

I agree to allow ITS / Next Byte to install any management agents onto my computer that will allow them to keep the University network secure.

I agree that by connecting to the University network, I may receive automatic updates to my computer from ITS to protect my computer and the University network. While these updates have been fully tested on the University Standard Desktop there is a small risk that it may cause problems with my system. I also agree that any problems caused by these will be at my cost.

I acknowledge that if this purchase was made without the consent of ITS then there is no internal ITS support for this Macintosh and all future work will be performed by NextByte at $130/hr.
