Guidelines for Vaccination

Note: The University's Policy and Procedures have recently changed. Over the next few months, the School's Guidelines, Procedures and documentation will be reviewed to reference the new University Safety Policy and HSW Handbook.

Vaccination

For eligible staff and students, the School offers various types of vaccination.

Before obtaining a vaccination, please read the following document "Guidelines for Vaccination".

The authorisation form should be completed and signed off by your supervisor prior to booking an appointment.

Vaccination Authorisation Form

 If you have made the payment for the vaccination yourself, authorised vaccinations can be reimbursed by the University.   Complete the Reimbursement Claim Form.
