I acknowledge and understand that:

• I have read and understood the statements contained on this form.
• I certify that the information I have provided is true and correct, and confirm that I will be held responsible for this.
• Failure to provide true and correct information may result in my being ineligible for the tour, and I authorise appropriate staff to verify the information provided here.
• I have a passport with at least 6 months validity from 30 June 2012.
• Whilst on the Study Tour I am an ambassador for the University of Adelaide and will behave accordingly.
• I am responsible for ensuring that I am meeting any official requirements (visas, travel permits, etc).
• Reasonable steps have and will be taken to ensure the traveller's health and safety is protected during the proposed travel in accordance with the University's travel policy.
• Refund Policy: If you decide not to undertake the Singapore Study Tour after making payment, you may seek refund for amount paid less all cancellation charges levied by external service providers such as Airlines, Travel agents and Hotels.
