Step 9 The ParchmentCollection of Parchment Postage of Parchment Changing Your Collection Option After the Graduation Presentation Ceremony Non-Collection of Parchment Replacement Parchment Reissue of a Parchment under a new name Academic Transcripts If you attend the ceremony, you will be presented to the Chancellor/Presider and your parchment will be handed to you on the stage. If you choose not to attend a ceremony, you must advise whether you or your authorised representative wish to collect your parchment, or have the parchment posted to your home address. Collection of ParchmentIf you have chosen to collect the parchment, then you or your nominated representative (with written authorisation from you), can collect the parchment from the Student Centre (Level 4, Wills Building) from mid April 2010. Photographic identification of yourself or your nominated representative will need to be presented at the time of collection. Postage of ParchmentIf you have elected to have your parchment posted, this will be actioned in mid April 2010, provided you have paid the appropriate postage cost: $15 for an address within Australia, or $25 for an address outside Australia. If you have not paid postage, please forward a cheque to the Graduations Office, made payable to The University of Adelaide. As your parchment is a valuable legal document, it is posted only by registered mail or by official courier to ensure safe delivery. Your parchment will be sent to the Home Address which the University has on its records for you. Please ensure that you advise the University via Access Adelaide in plenty of time if your home address changes. Collection Option After the Graduation Presentation CeremonyIf you had previously indicated that you would collect your parchment in person (after the graduation presentation ceremony), but you now wish to authorise someone else to collect the parchment on your behalf, please complete the Parchment Collection Proxy Form and return it to the Student Centre or give it to your proxy to present, along with photographic identification, at the time of collection. If you had previously indicated that you or your authorised proxy would collect your parchment (after the graduation event), but you now need the Graduations Office to post the parchment to you, please complete and return the Parchment Post Request Form. Non-Collection of ParchmentParchments not collected from the Student Centre will be returned to the Graduations Office and stored for a period of 7 years. The University takes no responsibility for the condition of the parchments stored in the Graduations Office, as parchments should not be stored in their plastic sleeves for an extended period of time. Should a replacement parchment be necessary, the cost will be borne by the graduate. Replacement ParchmentIf your original parchment is lost, destroyed or damaged, you can request a replacement parchment. The replacement will cost $100, and you will need to submit a Statutory Declaration signed by a Justice of the Peace stating the circumstances in which the loss, destruction or damage to the original occurred. The damaged original parchment will need to be surrendered to the Graduations Office. Replacement Parchment Form The replacement parchment will not necessarily look like the original as it will be issued in today's format for parchments, and it will state that it is a copy. Your name on the parchment will be printed the same as it was on the original. Reissue of a Parchment under a new nameIf you have changed your name, you can request your parchment to be re-issued in your new name. The replacement will cost $200, and you will need to submit documentary evidence of your change of name. The original parchment will need to be surrendered to the graduations office. Parchment Re-Issue Form Change of Details Form The re-issued parchment will not necessarily look like the original as it will be issued in today's format for parchments, and it will state that it has been re-issued.
