Forms and links

Below is a selection of forms and links which you may require for the purpose of graduating or receiving your testamur.

Student forms

  • Change of Personal Details form
    If you have a current student account, you can update your name by completing the change of personal details form online.
  • Change of Personal Details form
    If you no longer have an active student account, you can change your name by submitting the change of personal details form by email or hard copy.
  • Commonwealth of Australia Statutory Declaration
    Required for replacement and re-issue testamur orders.
  • Third Party Proxy Authorisation form
    If you wish to authorise a third party to collect your testamur from Ask Adelaide on your behalf, you must complete the Third Party Proxy Authorisation form and provide this to your proxy to take to Ask Adelaide, along with their own photo ID.
  • Access Adelaide Privileges form
    If it has been more than 12 months since you completed studies and you no longer have access to your Access Adelaide account, you will need to complete the Access Adelaide Privileges form to have your Access Adelaide account reactivated for the purpose of applying for conferral.
  • Invitation letter for visa purposes - guide
    The above document is intended as a guide for international students who are writing invitation letters for use by family members who are applying for a visitor visa to visit Australia.
  • GFP Online Portal
    If you are attending a graduating ceremony, you will need to login to the GFP Online Portal to order your Academic Dress and print your guest tickets.
  • My eQuals
    If you have been issued with a digital transcript,AHEGS and/or testamur, login to the My eQuals portal to view your documents.
  • Completion and conferral confirmation letters
    If you have completed or been conferred in your program, follow the link above if you wish to request a confirmation letter from Ask Adelaide.
