Video Submissions Form

This form must be submitted by the person who has acted as Supervisor for the video.

Before submitting this form, please ensure the followings steps have been completed/acknowledged:

The candidate’s repertoire information has been submitted via SCORE in the relevant enrolment.

The YouTube link is marked as Unlisted (not Private) and has been added as an attachment in SCORE.

The recording has taken place according to the instructions provided by AMEB SA & NT (includes verbally-stated date, time, enrolment key, exam name, supervisor name and candidate name), or the instructions provided by Rockschool (for Rockschool exams).

Sheet music and accompanying APRA Copyright Declaration Form is ready to upload via this form, for any Grade 8 or above recording or for any Own Choice/Extra List pieces not sourced from the manual lists (tick to acknowledge requirement, even where not applicable).

The performer is the candidate named on the enrolment and the performance was supervised and recorded in one take without teaching/coaching assistance.

For Comprehensive and Certificate of Performance exams, any scheduling requests for the Section III “mini exam” have been updated on the candidate’s enrolment in SCORE. Mini exams are scheduled on weekends between 8:30am to 12:30pm, usually within one fortnight of submission (tick to acknowledge requirement, even where not applicable).

I understand that the recording will not be scheduled for examination unless the conditions above have been met.

If you are a submitting a recording for a Grade 8 Comprehensive / Repertoire exam, Certificate of Performance exam or Level 3 Performance Evaluation, or an exam with Own Choice pieces selected from sources other than the AMEB manual list, please upload one PDF file of the full scores for all relevant pieces.
