Document Collection by Proxy
There are a number of documents that are made available for collection from Ask Adelaide, and there are often times when the owner of these is unable to collect the documents themselves.
In these cases, someone may authorise a third party (proxy) to collect the documents on their behalf. Proxy authorisation can be made at the time of placing your request (via the Online Shop for transcripts and letters, or via MyAdelaide for testamurs) but in the event that this has not occurred the below Third Party Proxy Authorisation Form can be submitted.
If you have been notified that one (or more) of the below documents have become available for collection from Ask Adelaide, but are unable to collect it yourself, this form has been provided to nominate a proxy to collect them on your behalf:

Document Collection for Former Students
If you completed your degree more than 1 year ago and your University login no longer works, please complete the hardcopy Third Party Proxy Authorisation form.
Your proxy must bring the original signed authorisation form and their photo identification to Ask Adelaide in order to collect your testamur.
Letters and Transcripts
Please email the Ask Adelaide team (firstname.lastname@example.org) to notify them of the authorisation. If you have received an email notifying you a document is ready, you can also respond directly there.
